Online Ordering Agreement

Please fill out our Online Ordering Agreement below and you will be provided with a printable version upon completion. Please notify us when you have completed testing and you will be sent an electronic invoice (after you have received your data file). Electronic invoices can be paid via credit cards or checks.


Online Ordering Agreement

This Agreement outlines Your rights and obligations to use the collegiate assessment instruments provided by Madison Assessment LLC ("Us", "We", "Our") to ("You", "Your").

  1. You can select from the Information Literacy Test ("ILT"), Scientific Reasoning Test ("SR"), Quantitative Reasoning Test ("QR"), Natural World Test ("NW"), US Society and Politics ("USSP"), and Test of Oral Communications 1 ("TOCS1") (each an "Assessment Test"). We will provide You access to:

    • Estimated number of TOCS1 test administrations:
  2. You agree to pay Us a fee of $8.00 per computer based Assessment Test (NW = $14) administered during the "Testing Window," which is the period between when the Assessment Tests commence and the End Date identified below. The minimum order is $200.00.
  3. You agree that Your use of the Assessment Tests ordered hereunder shall be governed exclusively by the End User License Agreement ("EULA") (the "Agreement") and by signing this Ordering Agreement You agree to be bound by all the terms and conditions contained in the EULA. Capitalized terms used in this Ordering Agreement that are not otherwise defined herein shall have the meaning given such terms in the EULA. In the event of any conflict between the provisions of this Ordering Agreement and the EULA, the provisions of the EULA shall prevail.
  4. The Student Opinion Scale ("SOS"), a 13 item measure of student motivation and demographics, is provided at no additional cost. This is optional.
  5. A schedule will be established for You to administer tests during the Testing Window. You will determine the testing schedule, and communicate it to Us no less than 2 weeks prior to the desired testing period. Any subsequent testing periods needed are to be requested at least 2 weeks before their start date. The Testing Window will end no later than
  6. You will be billed by Us at the end of the Testing Window. You agree to pay all amounts due upon receipt of invoice. You may request separate results reports relating to multiple testing periods, which will be billed to You at applicable standard costs at the time of Your request(s). The minimum billing is $200.00.
  7. We will provide a web address, and login information for Your Designated Contact (identified below) to use on Your scheduled testing date(s). You will be permitted to download a fixed format data file with the scored item responses. The content of the fixed format data file will include test identifier, student name, student identification number, total score, institution name, time started, elapsed time per question, and scored item responses (0, 1 for achievement tests; raw data for polytomous or Likert data).
  8. We will provide specifications for Your web browsers and their configuration.
  9. We will provide a contact person for establishment and recording of testing dates, procedures, and guidance through the end of the Testing Window.
  10. You will provide a contact person through whom all arrangements and guidance for the length of the agreement shall be distributed to members of Your institution. Your Designated Contact person is:

  11. IT IS YOUR RESPONSIBILITY TO MAINTAIN TEST SECURITY BY PROVINDING A SECURE TESTING ENVIRONMENT. A PROCTOR PROVIDED BY YOU MUST ADMINISTER ALL TESTS.

    Test security shall be achieved, at a minimum, through the following steps:

    • You will provide Us with scheduled dates for testing;
    • Access to tests will be controlled on Our server;
    • A customized URL will be issued for installation and use on testing computers;
    • Picture identification will be checked at Your testing site by Your proctor;
    • Identification numbers will be included with data sets sent to You by Us;
    • Use of cell phones, cameras and PDAs are strictly prohibited.

    I AGREE THAT TEST SECURITY SHALL BE ACHIEVED, AT A MINIMUM, THROUGH THE ABOVE STEPS IN ACCORDANCE WITH THIS SECTION 11.

  12. We will manage the file servers and will score all examinations. We will archive and maintain all data for three years.
  13. Additional analyses may be requested at additional cost to You.
